I accept most major insurance plans, though I no longer take Tricare or Cigna. Services may be covered in full or in part by your health insurance or employee benefit plan. You are responsible for finding out what your mental health benefits are, so please check your coverage carefully by calling the phone number on the back of your insurance card and asking the following questions:
Do I have mental health benefits?
Is Jon Mark Trusler in network?
What is my deductible and how much of it has been met?
How many sessions per calendar year does my plan cover?
Does out-patient counseling require pre-approval?
Don't panic - talking with insurance companies and navigating their automated systems can be confusing and frustrating, but if you have any problems I will be glad to help. However, it is your responsibility to make sure that you r visit is covered. Also, if you are not in agreement with the information on the explanation of benefits (EOB) it will be up to you to contact the insurance company and work things out. If an insurance claim must be filed a second time because I was given inaccurate information, the fee for the second claim will be added to your bill (it is typically $8.50 per claim).
If you intend to use your insurance benefits, you will be required to complete an Automatic Billing Authorization Form which allows me to automatically bill your credit card once I recieve an explanation of benifits (EOB) from your insurance company. Bank/debit cards are not acceptable for automatic billing. The client receives the EOB prior to the provider, so you will always know how much is being charged to your card prior to the transaction. If you don't receive an EOB within a few weeks of your visit, let me know. Your credit card information is encrypted and nothing is stored on my computer. Your credit card information is deleted upon your request, or 90 days after your final session (whichever occurs first). If you do not want to complete the Automatic Billing Authorization Form, or if you do not have a credit card, your options are self-pay or to find a therapist whose financial policies are a better fit for you.
Cash, personal checks, money orders/cashiers checks, bank (debit) cards, and credit cards (at office or through our website with Paypal) are accepted for payment. Payment is due at the beginning of the session. If you do not have your payment I will be happy to reschedule your appointment for a date that is more convenient for you. However, you will be charged $25 for a missed appointment. Although I love what I do, and I do this because I care and want to make a difference... this is also how I make my living. Please do not ask me to make exceptions.
If you do not show up for your scheduled appointment, or if you cancel less than 8 hours before your scheduled appointment, you will be charged a fee of $25. If you cancel your appointment less than 8 hours before you are scheduled, but I am able to fill that slot, you will not be charged the cancellation fee. Also, if your appointment is early in the day and you leave a voicemail for me the night before you are covered.
